Community health workers can provide supportive services to people living with dementia and their caregivers all within their surrounding communities. This support can take many shapes, such as providing home wellness and safety checks, assisting in navigating and accessing services, and delivering dementia-focused interventions. The ability of the community health worker to build trust and rapport with people living with dementia and their caregivers is key to their success.
